Below is a model for facilitators to help the athlete share their action plan with their circle of support. This model is covered in the video above.
Below are some examples of outcomes, actions and measures for various competencies. A note of caution, this is not an example of an action plan as you wouldn’t normally have this many competencies on a plan because you would spread your focus too thin.
